Set up time will be based on booth location. We will try to accommodate your preference.

Additional opportunities to support BMCF (these are optional, have no bearing on your acceptance to the show, and will not change your payment today):
 I will donate a raffle prize. I understand that this is a voluntary donation and should be submitted the morning of the show.
 I am interested in purchasing a 50/50 Raffle ticket. Please contact me separately at my submitted email so I may pre-purchase a 50/50 Raffle Ticket.

 1 Booth (10x10)  $55.00 ($80.00 after July 1, 2019)
 2 Booths (10x20)  $100.00 ($150.00 after July 1, 2019)
 3 Booths (10x30)  $145.00 ($225.00 after July 1, 2019)
 Corner location premium (limited)  $ 15.00
 Electricity (limited)  $ 15.00

**Personal Checks accepted only with printed and mailed applications and if dated and postmarked on/before September 20. After this date, payment must be in form of Cashier's Check, Money Order, Cash, or Online application with Credit Card.**

 I verify that I created and/or have the right to display all of the work I have applied to exhibit during the 2019 Batavia Mothers’ Club Foundation Autumn Craft and Vendor Show.
 I have submitted photos for my crafts with complete Name, Business, and contact information as entered on this form. Submission must come from email listed on this form. Application is not complete without photos.
 The undersigned hereby expressly agrees to indemnify and hold harmless The Batavia Mothers' Club Foundation from all claims, loss, damage or causes of action of any kind of nature whatsoever which may arise as a result of the occupancy and use of the herein described premises by the undersigned. Such indemnification to include damage to the personal property of the undersigned, its agents, or employees or persons attending such function.
